Money Request Form

The coaching staff is dedicated to giving athletes the tools, training, and opportunities they need to excel both on and off the field. Providing high-quality coaching, travel, equipment, and facilities requires significant resources. Financial support helps bridge this gap, allowing players to focus on developing their skills, building character, and proudly representing the program. With community support, these athletes can reach their full potential and create memories that will last a lifetime.

Funding Request Terms & Conditions:

• This request must be submitted by the designated coach.
• The Athletic Director must be informed of and acknowledge this request.
• All approved funds must be used strictly for the purpose outlined in this request.
• Any modification to the intended use of funds must be submitted in writing and approved by the Booster Club prior to expenditure. Approval may require a formal vote of the Booster Club Board.
• Any unused funds, or funds not used in accordance with the approved request, must be returned to the Booster Club in full.
• The Booster Club reserves the right to deny, revoke, or request the return of funds if these terms are not followed.
• Submission of this request constitutes acknowledgment and agreement to all terms and conditions outlined above.
